Type of a scam Fake Invoice/Supplier Bill

Our church received a form specifying that we should pay $*** to "STATEMENT OF INFORMATION SUBMISSION", so that they could file our biennial Statement of Information. A copy of our current California statement information was printed out and included with the "bill".

The form clearly targets churches.

Type of a scam Counterfeit Product

We received a mailing that pretended to be the biannual request for "Statement of Information" that all nonprofits have to file with the Secretary of State. This filing usually costs us $**. The "Statement of Information Submission" form we just received has no phone or website listed and even says in small print that it is "not approved or endorsed by any government agency." The included envelope goes to a PO box, not to any government agency, and the required fee was $***. They clearly accessed our previous records because they sent a copy to us to make the invoice look more convincing.

Type of a scam Fake Invoice/Supplier Bill

Cornerstone Christian Community El Buen Pastor Assembly of God a non profit organization (church) received a invoice to pay $150 payable to STATEMENT OF INFORMATION SUBMISSION Section 6210 on a form that wanted us to write all the information of our church and officers names and addresses. I attached the copy. Spelling on this invoice is incorrect in some words. When you scan the bar code it shows ABCD 1234 abcd.
