Type of a scam Sweepstakes/Lottery/Prizes

A local resident received a letter from Opportunities Unlimited Publications, Inc., (OUP), stating the resident had been notified several times and OUP had not heard from him and now his prize was in jeopardy. Resident was asked to return the standard Qualifying Question, included with the letter, and the "specially discounted $5 registration fee." This would provide resident "FULL APPROVAL as Qualified Nominee for the $15,000.00 Grand Prize." Names included on the letter are John Winstead, Prize Director; Thomas Moore, judge; Mitchell Wagner, judge; Mary Kelley.
