How are you doing today? Survey Payment was delivered to your mailbox by USPS courier service on Wednesday (06/01/2016). It contains a check which covers your allowance for your evaluation service for this survey and funds for evaluations you will be conducting.

Acknowledge that you receive your package ( Check payment and Instruction letter).

In my mailbox there was a Priority mail express envelope with a letter and a check for $1,570.00. The envelope label, the letter, and the check are pictured in the attached image.
