On Nov. 6, 2015 I recd an Annual Solicitation Form from Nashville, TN. Review the accuracy of the preprinted corp. name/address and make any changes necessary. Verify: Name of each stockholder; name of all members of Bd of Directors; title & name of an officer; name, email & ph no. of the person to contact. Provide valid pmt method; sign the form to verify validity of info provided; return entire completed form w/pmt of $125. ScamPulse.com cautions businesses in completing the form & sending $125 to Corp. Records Svc as the co. is not a govt agency & address listed on the mailer is that of a commercial mail receiving agency & not the physical address for the co. Altho, TN corporations are required to file an annual report, this can be done online (Sec of State’s website) for a nominal fee of $20.
