Our family practice received faxes from a place that is called "The Pharmacy" to sign prescription requests authorizing numerous pain prescriptions for two separate patients. There is no valid reason for the prescription requests, no facility address for "The Pharmacy", no phone number for the facility and the requests are marked as urgent requests. There are more than 6 requests for pain prescriptions for each of the patients.

We received 3 faxed requests for 2 different patients for a huge list of items they are wanting the provider to ok and fax back to them. Fax # listed is 1-703-214-3850. One is an actual patient, the other I can’t find in the system. The items they want ok’d are:

lidocaine 7% cream, Doxepin 5/5 cream, Diclofenac 1.5, Triamcinolone, Fenoprofen, Chlorzoxazone tab, Kamdoy skin emulsion, Calcipotriene cream, Folika, Prodigen, Omeprazole

Received messages 10-25 and 10-26 from The Pharmacy stating that my doctor had faxed them a prescription and they needed more info before they could mail it to me. I called them back at 513-952-8848 to find out what they were calling about and the man on the phone repeated that my doctor (Dr. Carol Marino) had faxed a prescription and he needed more info before it could be sent. I told him I have not seen my MD in over a year and not on any medications. I asked what medications and he told me 2 medications and I asked what they were for and he told me they were for the skin. I told him that these medications would not be for me. I called mY MD office and explained what I had just experienced and my MD office had never heard of The Pharmacy and had not sent any prescriptions to anyone under my acct.
