Type of a scam Phishing

The mail came addressed to me at my residence with a return address showing "Senior Benefits" P.O. Box 2910, Kennesaw, GA, 30156-9843. Inside was a small form to fill out, with the year 2020 on the upper right hand side to make it look like an office IRS or Franchise Tax Board form. The header on the form reads "2020 BENEFIT INFORMATION FOR CALIFORNIA CITIZENS ONLY." States in a pp at the top "As a resident of California, you are entitled to more benefits not provided by government funds. You now have access to a 2020 state-regulated life insurance program which will pay 100% of all final expenses up to $35,000. Return this postage paid card within 5 days to request this new benefit information." The form then has space to fill out Name, age, Spouse’s name and age, Street address and phone number. There is a bar code at the bottom with ref EQ14736B104775. I’ve covered up my name and address in the image.
