Type of a scam Counterfeit Product

I was using the internet browser on Friday, February 17, 2017 and a pop up screen appeared and it said that our computer was infected with a virus, malware and Trojans from foreign country and the computer locked up. The pop up had a Microsoft logo on it and said to contact 844-443-0777 right away and they could assist us. Remote Help Online person said that they were listed with the BBB. They showed me a fake screen that showed how my computer was being hacked and the IP addresses that were being used since October 6, 2016. I believed what he was saying and asked if they could help. I gave him permission to remote into my computer so he could resolve the issue. I agreed to have him help me and he said that it would cost $199.99 a year for them to help me with eliminating the virus, malware and Trojans from my computer and put a firewall on it for a year and called it their Gold Plan. They had me fill out my billing information as attached document shows below. The company forged my signature. It is not my signature. I became suspicious of this and called Microsoft on Monday, Feb. 20th and found out that it was a scam and Remote Help Online was actually installing viruses, etc. to control my computer without my knowledge. A Microsoft tech helped me on Monday, Wednesday, and today (Feb. 20, 22 and 23, 2017). Today the 23rd, the Microsoft tech. had to completely uninstall my whole Windows 10 Operating system and reinstall it. I am hoping it resolved the issue and deleted all the scammers control over my computer.
