I received a call from "Prosperity Wellness" from the telephone number (937) 960-1013.

The caller indicated that I had requested a "patient advocate" contact by check-marking a box on a form I filled out at a medical provider I visited within the past 6 months. The caller further stated that my request was due to body pain and I was searching for an alternative to prescription narcotics.

The caller asked for me by name. I redialed the number from caller ID and the person that answered knew my name, likely from their caller ID.

The cause for my suspicion is that Prosparity Wellness is located in Washington state. The telephone number on caller ID was local to the Dayton, Ohio area. The number on caller ID is not associated with any registered user when I searched on the internet.

I called Prosparity Wellness and they confirmed that no one would have tried to contact me as a potential client and that their client base is only in their area in Washington state.
