Neither my doctor or my accountat have ever heard of this and both believe that this is a scam.
Why would the NDCA require your SIN other than for identity theft?
The CRA offers Disability Tax Credits that total this much for children, usually $15-20k for adults. You need to authorize a representative with the CRA using a SIN number. Not sure why your accountant hasn’t heard of that or your doctor hasn’t ever filled out a disability tax credit certificate.
