Type of a scam Tech Support

I got a call on my landline from a Ronald Wilson at Lifeform. He called from 760-695-4365. He wanted to give me a $300 refund from their Wells Fargo account. He said they could only talk to me with the computer on. He had me set up a Lifeform account on the computer. He sent me a form via the computer and said I needed to fill it out. So I did. It asked for my address, cell phone number, email and birth date. I could see them going through my files.  Ronald then told me he was a hacker.
